Sign up below for each recipient. Answer the questions below and schedule your vaccination date.

Pfizer, Moderna, and Johnson & Johnson available.

 

  • You must provide your insurance card for your vaccination. If you do not have insurance, you must provide your driver’s license.

    • At least 6 months after Pfizer or Moderna

    • At least 2 months after J&J

  • You may have a preference, but you can get any booster shot.
